This is for New Clients. I will be sending you an online questionnaire to your email to complete prior your appointment.

  • Then purchase the coaching call. (No refunds, and 48 hour reschedule / cancel notice required).
  • Complete the online questionnaire and email me any pertinent info regarding your health to [email protected]
  • Schedule your appointment. Available Monday through Thursday 12:30, 2:30, 4:30, 6:30 MTN USA time
  • You will get emailed confirmation of the coaching call purchased, online questionnaire and reminder text of your call
  • I will call you at the time of your coaching call.

48 hour reschedule or cancellation notice please. If cancel or reschedule with less than a 48 hour notice, the appointment does not carry over. There will not be a refund if not give 48 hour notice. Please schedule appointments you feel really good about keeping. I am dedicating 2 hours of my day to prepare and talk with you.
